To request that our office directly provides verification of your North Carolina license to another state, please complete the below fields and click submit. Note, there is no fee for verifications.
Please note, we no longer complete specialized forms for other states, nor do we mail paper forms to physical addresses. Our standard verification letter not only expedites the process but should satisfy the verification of licensure. Should you encounter an issue with this policy change, please have the state reach out to us directly at info@ncbdn.org.
You may submit up to five states at a time and you must provide contact information for all states requested.

Note: Our office staff cannot fulfill a verification request like this via voicemail or phone; it must be requested in writing via this contact page or via Postal Mail/USPS.
You also have the option to generate, save, or print a letter of license verification that may meet your needs by accessing the NCBDN Licensee Gateway. Once logged in, click on your license, and on the following page under “Options,” download or print a License Verification Letter.
